For Brands Hatch will these be available at sign on or do they have to be submitted in advance?

It used to be that you'd phone up, they'd post/fax/email you a form, and you'd post/fax/email back a form before the event (as well as taking a copy on the day just in case).
MSV these days has pretty much universally, for club racing, changed to an "on the day" format. It won't be available at signing on, but you will be able to go to the Circuit Office and get a form sorted for you.
Allow extra time, as chances are you'll be in a queue of people waiting for a camera form, as well as any photographers signing in with their media accreditation forms... So queue once for the camera, queue twice for signing on and queue thrice for scrutineering. All at pretty much the same time (maybe MSV will make it a bit easier and not have signing on the scrutineering quite so close together).
As ever, the best way would be to get on the blower and speak to Brands directly, rather than asking people what has happened in the past. They might have a completely new system for 2010 that nobody knows about.
Edit: You must also have the camera system fitted onto the car at scrutineering, and a VIDEO stamp on your pass ticket. Failure to do either could result in you being plucked out of the assembly area and told to sort it out. Which could mean you miss qualifying. Or a race. Or both. It also needs to be physically attached. Velcro is out. Cable ties are probably allowed (certainly as a backup) but would be rubbish as you'd get too much camera movement. Suction cups aren't allowed. You need to physically bolt the camera onto the car where possible.

Spoke to them directly, they have emailed me the form.
Trying to make a camera cradle that will absorb the bumps, my last attempt was horrible with the picture being wavey.

I know how you feel... I spent several hours at the weekend milling aluminium into a mounting block for both my cameras (forward in HD, rearview in SD), but chances are they won't be perfect at the first go...

Just TESA tape it to the roll hoop with a small cube of foam in between to dampen vibrations.
Works a treat and has passed scrutiny no problem from F1 support races to Cadwell Park clubbies.

Im going to use some padding from an old PC palm rest together with a cradle to absorb any vibrations.
Something similar to these, but I think they are using neoprene pads or similar.
